 When Mayhem is defeated, his hair changes and he thanks the player for freeing him. We are lead to believe that that was his true form, as he would then possess or attempt possess individuals such as Hollow.

Who was that? What happened? Why'd he thank us? Was that not Mayhem and just Mayhem possessing someone? Was Mayhem being forced by War to serve him?

 While Mayhem's original identity wasn't revealed, the three lieutenants of War were indeed being forced to serve. This treacherous act is part of what rendered the prior demigod unworthy, making the profane altar necessary to cling to their power. Overcoming them or War proved able to free them, although Carnage would prove the most resilient in this regard and would eventually be reclaimed for one last bid for power. Panic was not reclaimed or 'recycled' due to being contained, while Mayhem had been freed while War could do nothing to counteract his escape.
