DESCRIPTION
This scythe is wielded by those that separate souls from their mortal bodies, severing their connections to the realm of the living. It can drain a bit of a foe's soul to gain a temporary increase in power, dealing more damage!

From the Nowhere quest: Reaper of Souls. Kam says:

Darkness Ranged Scythe, -3 BtH lean and 1/3 base lean. 20% proc special. Current thought is that when the special fires, the weapon gains a piece of the foe's soul. This makes it deal +12.5% damage on the following 2 turns (5% MC, 20% special so /0.2, that's 25%, effect split over 2 turns so 12.5%). If you get the special 3 turns in a row, then you'll get this Normal Damage (2 turn counter is added), +12.5% damage (initial counter is decremented, since a turn remains the final turn on the counter now deals +37.5% damage), +37.5% damage (counter 0s out, since counter is now 0ed out, you now get the standard 2 turns of +12.5% damage for the next 2 turns). Glad to change to something else if another idea comes up though.

IMR adds:
FSB: Weapon now functions like the Hydromancer's Bloodblade. You pay the listed HP per turn to get a +30% damage boost.

So...

You get the special on an arbitary turn 1 and get the boost for turn 2 and turn 3 for a total of +12.5% damage. Then the special happens again on turn 2 and you get a second boost and is going to apply during turn 3 and turn 4 with you having a total boost of +25% immediately after the second boost starts being applied. Then the special happens again on turn 3 and you get another boost that will last during turns 4 and 5 for a total boost of +25% imediately before and during the execution your attack and +37.5% after said attack. Then once turn 4 comes around you lose the boost that was applied on turn 1 even before you attack but you'll still have the boosts from both turn 2 and turn 3 at this point.

So shouldn't that add up to a total boost of +25% on turn 4 before and during your attack rather than to one of +12.5%?

Turn 1: Special happens. It does normal damage and sets up a +12.5% boost for 2 turns.
Turn 2: Special happens. It does +12.5% damage and increases the boost. The boost currently has +12.5% damage for 1 turn, so it increases that to +37.5% for 1 turn.
Turn 3: Special happens. It does +37.5% damage and increases the boost. The boost has 0 turns left, so it sets up a new boost at +12.5% for 2 turns.
