Crystal Petey is a permarare, but if you have it that's also an option. Bat Swarm guests from the Vampire subrace armors also seem to inflict blind. For miscs, there's the permarare Frostval Merc's Smoke Grenade if you have that somewhere. There's also The Brilliance of Daw. I probably missed a bunch of stuff though.
