he has no custom title how does he have custom avvy?

ZanpakuTô is a retired ArchKnight. Retired mods/AKs retain their avatar and title privileges, and aren't required to exercise either one. It's likely that he chose to use a custom avatar and use a normal member title.

Also (and I'm only adding on because it's potentially useful info), titles automatically reset to "Member" when an account's group memberships are changed, so you'll see a fair amount of older members who used to have titles but lost them due to the elimination of a group or something.
